Qualifications

• Active Nurse Practitioner license in New York.
• Medicare and Medicaid credentialed.
• Strong clinical assessment and communication skills.
• Telehealth experience is a plus.

Responsibilities

• Conduct patient evaluations and follow-up visits through a telemedicine platform.
• Diagnose, treat, and manage a variety of acute and chronic conditions.
• Develop and implement individualized treatment plans.
• Maintain accurate and timely electronic medical records.
• Collaborate with physicians and clinical staff to ensure exceptional patient care.
• See approximately 25 patients per day.
