Job summary

TriSearch seeks a nurse practitioner or experienced clinician for in-home wellness assessments under insurance plans. The role involves conducting adult/geriatric evaluations, documenting findings, and coordinating with PCPs and care teams.

Flexible scheduling and mileage reimbursement are offered. Must hold RN and NP licensure in the practicing state with active board certification; new grads welcome with valid certification.

Qualifications

• BSN preferred; MSN/NP licensure required.
• RN and NP licensure in state of practice.
• Board certification in Adult/Family/Geriatrics or NP specialty.
• Current CPR certification.
• Multistate licensure preferred.
• MS Office proficiency.

Responsibilities

• Perform adult/geriatric assessments and document according to guidelines.
• Collaborate with physicians, case managers, social workers, family members, and care teams.
• Coordinate with PCP on education and follow-up.
• Provide services via Home Visits and Skilled Facility Visits.
• Mileage reimbursement provided for travel.
• Maintain PHI and comply with OSHA and nursing board standards.

Pay:From $90.00 to $115.00 per assessment

Wellness visits- in-home (insurance) assessments

1) LOCAL PRN:

* 20 in-home(1 hour) HRA's per month- Health Risk Assessments for the elderly

* Flexible schedule- you pick your days and hours(M-Sun available 8am-7pm)

* 90$ per visit and we reimburse mileage!

* new grads welcome! - must have an ANCC, AANP, or AACCN valid certification

Earn from $1,800 up to $9,000 per month (based on an average of 20-100 completed visits per month and pay of $90.00 per completed assessment).

What to Expect:

• Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to our guidelines and protocols
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up
• Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits
• Mileage Reimbursement plan

ESSENTIAL JOB FUNCTIONS

• Perform screenings including medical history, physical exam, medication reconciliation and education.
• Effectively identify, evaluate, and address the concerns of the member, employee or participant.
• Order and interpret diagnostic imaging, and laboratory results and provide clinically appropriate interventions, additional tests, or referral as needed.
• Administer and order point-of-care and lab-based testing
• Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary.
• Work collaboratively with a variety of stakeholders including risk & quality teams, staffing, operations, physicians, case managers, social workers, care givers and ancillary medical personnel.
• Participate in all mandatory compliance and other training programs, seek guidance for compliance related concerns and adhere to all applicable laws, regulations and Matrix policies and procedures.
• Create and maintain clear documentation according to clinical standards, guidelines, and protocols.
• Maintains positive control of all PHI information and compliance with OSHA and State Board of Nursing.
• May act as a preceptor for new providers.
• Other duties may be assigned

POSITION REQUIREMENTS:

Educational Requirements:

• Bachelor of Science in Nursing preferred.
• Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure.
• Current RN and NP licensure in the state of practice to include prescription authority or the ability to obtain prescriptive authority.
• Board certification by AANP, AACN or ANCC, in Adult, Family Practice Geriatrics or Adult-Gerontology Primary Care, or ACNPC-AG (Adult Gero) Acute Care NP, or ACNPC (Adult) Acute Care NP.
• Current CPR certification.
• Multistate licensure preferred.

Knowledge, Skills and Abilities:

• Demonstrated ability to: Perform comprehensive geriatric, adult physical exams, based on scope of practice for licensure and certification.
• Possess a valid state driver license and proof of coverage, and meet company’s driving and automobile liability insurance standards.
• Ability to travel to assigned locations via public transportation including airplanes.
• Proficient computer skills, including e-mail and Microsoft Office.
• Effective written communication skills including but not limited to writing reports and correspondence.
• Strong verbal communication and effective listening skills, including the ability to present and maintain a professional, empathetic demeanor at all times.
• Demonstrated ability to prioritize work, meet deadlines, and use time efficiently.
