Job summary

OnSite Medical Solutions is seeking a Nurse Practitioner to provide high-level medical and mental health care for older adults in nursing homes, assisted living facilities, SNFs, and home settings.

You will have administrative support and clinical mentorship while developing an autonomous practice with a flexible schedule and competitive compensation. A collaborative team approach and ongoing mentorship are emphasized.

Qualifications

• Master of Science in Nursing with NP certification (FNP/ANP/AGNP/AGACNP/AGPCNP/ACNP).
• Active South Carolina license.
• DEA registration.

Responsibilities

• Round at assigned facilities and manage acute, subacute, and primary care needs of older adults.
• Collaborate with OnSite team, nursing home staff, residents and families.
• Follow up with patients and assess effectiveness of interventions.
• Provide education on best practices.
• Promote non-pharmacological interventions where possible.
• Document patient care accurately and thoroughly.

• Master of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification
