Job Opportunity: Nurse Practitioner or Physician Assistant - Brooklyn, NY

$40,000 Student Loan Repayment or $40,000 Sign-on Bonus for individuals who have not previously participated in this program.

Optum is seeking a Nurse Practitioner or Physician Assistant to join our HouseCalls team in Brooklyn, NY. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you'll provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks, and coordinate follow-up care to improve health and well-being.

Our integrated medical teams are innovating in healthcare by delivering high-quality medical service, compassionate care, and industry-leading solutions to vulnerable populations. We address physical, mental, and social needs, helping patients access and navigate care anytime and anywhere, creating a seamless health journey.

This role requires 80% travel and overnight trips, with travel by vehicle to counties including Brooklyn, Queens, and Westchester. A map will be shared upon request. Travel involves overnight stays approximately 80% of the time, around 10 weeks per quarter, plus 2 weeks in the home county.

In this role, you will:

  1. Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform screenings and point of care testing as appropriate.
  3. Identify diagnoses for care management and treatment.
  4. Document medical conditions and assessment findings.
  5. Communicate assessment findings to the patient's PCP.
  6. Identify urgent situations and intervene as necessary.
  7. Educate members on disease processes, medications, and compliance.
  8. Travel up to 85% as needed.

Qualifications:
  • Active NP or PA license or ability to obtain by start date; license must be free of disciplinary actions.
  • Active national certification (ANCC, AANP, NCCPA, etc.) or ability to obtain it.
  • Physical ability to perform assessments and move equipment.
  • Driver's license and reliable transportation for travel.
Preferred Qualifications:
  • 1+ years of clinical experience.
  • Additional licensure in other areas if requested.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health or home visit experience.
  • Ability to travel an additional 5% for incentives.

The salary range is $89,800 to $176,700 annually, based on experience, education, and location. Benefits include comprehensive packages, incentives, stock purchase options, and 401k. Employment is contingent upon passing a drug test and maintaining active employment status for a minimum of 12 months; full student loan repayment benefits require a 36-month commitment within the role.
