Job summary

Northeast Healthcare Recruitment is seeking an experienced Nurse Practitioner or Physician Assistant to provide autonomous, primary care in skilled nursing facilities on a part-time basis. You will manage your own panel, conduct annual wellness visits, and address acute issues with strong clinical judgment.

Expect 16–20 patients daily, with 6-hour morning rounds, no on-call or overnight duties. You’ll document from home when possible and receive a productivity-based compensation structure plus

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients a day in SNF, including wellness visits and H&Ps.
• Provide primary and episodic care and manage chronic conditions.
• Coordinate with collaborating physicians, specialists, and facility staff.
• Review meds and orders and ensure seamless care; document thoroughly in EHR.
• Join care conferences with families as appropriate.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team part time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues.
