Job summary

Northeast Healthcare Recruitment invites an experienced Nurse Practitioner or Physician Assistant to join a skilled nursing facility clinical team. This full-time, autonomous role centers on continuity of care, managing your own panel, and building lasting relationships with residents and their families.

You will see 16–20 patients per day, perform wellness visits and acute care, review medications, and coordinate with physicians, specialists, and facility staff to keep care seamless.

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted NP or PA license in the operating state.
• Master's degree in Nursing (NP) or from an accredited PA program.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients daily, including long-term chronic care and acute issues.
• Coordinate with collaborating physicians, specialists, and facility staff to maintain seamless care.
• Perform annual wellness visits, H&Ps, and medication reviews.
• Engage with families and participate in care conferences.

Nurse Practitioner or Physician Assistant — Skilled Nursing Facility

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.
