Northeast Healthcare Recruitment, Inc. is seeking an experienced Nurse Practitioner or Physician Assistant for a part-time role in skilled nursing facilities.

You will manage your own patient panel, provide primary and episodic care, and build lasting relationships with residents and families in an independence-driven position. Expect 6-hour morning rounds with 16–20 patients daily, no on-call, and charting from home.

Qualifications

• Board certification required (AGNP, FNP for NPs; PA-C for PAs) — active and unrestricted.
• Unrestricted NP or PA license in the state of operation.
• Master's degree in Nursing (NP) or PA program required.
• Experience in skilled nursing or long-term care strongly preferred.
• Strong independent clinical judgment and ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients daily, including annual wellness visits and H&P.
• Manage chronic and co-morbid conditions and respond to acute changes.
• Review meds and orders and coordinate with physicians, specialists and facility staff.
• Stay in touch with families and participate in care conferences as appropriate.

Nurse Practitioner or Physician Assistant — Skilled Nursing Facility (Part Time)

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team part time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
