Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted NP or PA license in the state of operation.
• Master's degree in Nursing (NP) or from an accredited PA program — required for the credential.
• Long-term care or skilled nursing experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients per day, mix of long-term chronic care and acute issues.
• Perform annual wellness visits and H&Ps, manage chronic and co-morbid conditions.
• Respond to acute changes and review meds and orders.
• Coordinate with collaborating physicians, specialists, and facility staff to keep care seamless.
• Stay in touch with families and participate in care conferences when appropriate.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team part time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.
