Job summary

Northeast Healthcare Recruitment, Inc. is seeking a Nurse Practitioner or Physician Assistant to join a Skilled Nursing Facility team in a full-time, autonomous capacity. You will manage your own patient panel, conduct 6-hour rounds, and chart from home after visits, with no on-call requirements.

You’ll see roughly 16–20 patients daily, handling wellness visits, chronic care, and acute issues while coordinating with physicians, specialists, and facility staff to ensure seamless care.

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the corresponding credential.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients a day, including long-term chronic care and acute issues.
• Perform annual wellness visits and H&Ps, manage chronic and co-morbid conditions.
• Respond to acute changes and adjust treatment plans as needed.
• Review medications and orders; coordinate with collaborating physicians and facility staff.
• Stay in touch with families and participate in care conferences where appropriate.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.
