Responsibilities
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. The Comprehensive Health Assessment (CHA) provides insight into a member’s overall health and helps close multiple care gaps in a single visit. Our providers:
- Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up, and documentation per Matrix guidelines
- Administer and order point-of-care and lab-based testing as needed
- Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary personnel
- Coordinate with the Primary Care Physician (PCP) on patient education and follow-up
- Provide services at the member’s residence, which may be in their home or Skilled Nursing Facilities

- Flexibility – Assessments scheduled based on your availability. Shifts include Sundays–Saturdays, 8 am–7 pm, with daytime, evening, and weekend options.
- Competitive Pay – Compensation based on the number of assessments completed. Earning potential increases with availability and travel to see members outside your home location. Monthly averages of 20–100 visits may yield $1,800–$11,500, based on a per-assessment rate of $90–$115 depending on state of residence.

- Additional Licensure – Support obtaining additional state licensure and credentialing in neighboring states or other desired locations.
- Full-time Opportunities – PRN providers can transition to full-time roles at home location or region/state and may receive a full-time sign-on bonus.
Qualifications
Skills & Experience That Shine
- Master’s Degree or commensurate experience with NP or PA licensure
- Current NP or PA licensure in state of practice with prescription authority or ability to obtain prescriptive authority
- Board-certified by NCCPA, AANP, ANCC or AACCN in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year experience as a Nurse Practitioner or Physician Assistant preferred; new grads encouraged to apply
- Valid state driver’s license and proof of auto insurance for the state of residence
- Strong computer skills and familiarity with electronic health/medical records software
- Excellent verbal and written communication skills
