Position Overview

During visits lasting up to one hour, providers review members’ health, medical history, medication adherence, social environment, and other risks. This comprehensive assessment improves care quality and helps close care gaps efficiently.

Responsibilities

What to Expect:

• Conduct assessments including medical history, diagnosis, treatment, education, and referrals
• Collaborate with healthcare team members and family
• Coordinate with Primary Care Physicians for follow-up
• Provide services in various settings: Home Visits, Skilled Facility Visits

Position Details

Type: PRN - Flexible schedule
Pay: From $1,900 to $9,500/month based on completed assessments (average of 20-100 visits/month at $95 per assessment)

Location: Camden, ME, and community environments
Hours: Days, evenings, nights, weekends, flexible
Benefits: Non-benefited except for 401(k) and voluntary benefits; mileage reimbursement included

Qualifications

• Master’s Degree or equivalent experience with licensure
• Current RN, NP, or PA licensure with prescriptive authority
• Board certification in a Matrix-approved specialty
• Current BLS, ACLS, or CPR certification
• 1+ year experience preferred; new grads encouraged
• Ability to travel within or outside the state
• Strong computer and EMR skills
• Excellent communication skills and a positive attitude

Travel Requirements

• Travel to meet patients at their locations
• Valid driver’s license and insurance
