Job summary

Trinity Health Of New England Medical Group in Springfield, MA seeks an advanced practice registered nurse or physician assistant to join our Adult Medicine Primary Care team.

In this full-time outpatient role, you will work with a collaborative team, access on-site labs and imaging, and use Epic EMR to coordinate patient-centered care. Qualifications

• Licensed to practice as APRN or PA in the relevant state ( Massachusetts preferred ).
• Experience in outpatient adult medicine is a plus.
• Proficiency with Epic EMR or similar systems is desirable.

Responsibilities

• Provide outpatient primary care to adults in a team-based setting.
• Collaborate with physicians and other clinicians to develop and implement care plans.
• Utilize Epic EMR to document visits, orders, and follow-up plans.

• You will have the resources needed to provide exceptional care on site, including labs, radiology services, medical specialists, and a hypertension clinic.
• The successful candidate should be licensed to practice in Connecticut and be a team player with an interest in providing full-spectrum care for adult patients.
