Job summary

Ennoble Care is seeking a Full-time Nurse Practitioner or Physician Assistant to provide primary and palliative care in the Port Charlotte, FL region. Clinicians travel to patients' homes, delivering continuous care for chronic conditions and limited mobility.

You will perform exams, build care plans, and work with a team to support patients, families, and caregivers. A Florida NP/PA certification and 1–2 years in primary care with geriatrics are required.

Qualifications

  • Board-certified NP or PA with Florida credential.
  • Strong clinical skills to assess and diagnose.
  • 1–2 years of primary care with geriatric patients; hospice exp. preferred.

Responsibilities

  • Provide high-quality primary and palliative care in Port Charlotte, FL.

Purpose:

We are looking for an experienced Full-time Provider (Nurse Practitioner or Physician Assistant) to provide primary care to our patients in the Port Charlotte, FL region. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

What You'll Do:

  • Provide high quality care to primary and palliative patients in and around Port Charlotte, FL
  • Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care
  • Effectively communicate with office staff and clinical operations across various internal platforms
  • Provide high quality, patient centered care to a panel of patients
  • Attend quarterly medical staff meetings
  • Develop impressions, diagnosis, and treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
  • Order diagnostic testing as medically necessary
  • Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:

  • Board Certified, with a valid NP or PA certification in the State of Florida
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Positive demeanor and strong people skills
  • Ability to confidently problem solve and operate independently
  • 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Salary Range:

  • Full-time: $105,000 - $130,000+ (depending on experience and other incentives)
  • Providers are paid per visit
