Position: Nurse Practitioner or Physician Assistant (Part Time, 20 hours/week)

Location: New London County, CT

Join Optum Home & Community Care, part of UnitedHealth Group, to deliver holistic health care addressing physical, mental, and social needs. Be part of our Senior Community Care (SCC) team, providing care at home, nursing homes, and assisted living facilities.

Compensation & Benefits
  • $40,000 Student Loan Repayment or $20,000 Sign-on Bonus (for eligible participants)
  • Salary range: $104,500 - $156,000 annually (full-time equivalent)
  • Flexible scheduling, no on-call, weekends, or holidays

Primary Responsibilities
  • Conduct assessments and manage acute or chronic conditions
  • Manage short-term rehabilitation episodes
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with interdisciplinary team
  • Discuss advanced care planning
  • Provide patient and caregiver education and counseling
Qualifications
  • Certified APC (NP or PA) with active, unrestricted license in Connecticut
  • Graduate of accredited MSN or DNP program
  • Board certified (AANP, ANCC, NCCPA)
  • Active DEA license or ability to obtain
  • Ability to perform physical tasks (e.g., lifting, navigating stairs)
  • Reliable transportation and driver’s license
Preferred Skills & Experience
  • Over 1 year of clinical experience, preferably in long-term care
  • Experience with complex behavioral, social, or functional needs
  • Knowledge of geriatrics and chronic illness
  • Proficiency in electronic medical records
