Job summary

Allegheny Health Network seeks an experienced CRNP/PA to deliver primary healthcare in non-acute or chronic conditions within a physician office setting. The role includes coordinating care with families and interdisciplinary teams and performing wound care and pharmacologic therapies.

Weekend and on-call coverage may be required. The candidate will conduct health assessments, develop care plans, order tests, document records, and educate patients on prevention and treatment options.

Qualifications

• Master's degree MSN from the practicing State Board of Nursing Approved Master's Program.
• National Board Certification for Nurse Practitioners.
• Practicing-state licensure as a Nurse Practitioner.
• Master’s degree in Physician Assistant Studies.
• PA-C or board eligible with NCCPA recertification within 90 days of hire.

Responsibilities

• Provides appropriate patient care including recording medical history and physical assessment, developing therapeutic plan, ordering diagnostic tests and documenting medical records.
• Coordinates patient care with patient, family, community health services and interdisciplinary team; involves physician in plan of care and clinical decisions beyond the CRNP’s scope.
• Provides monitoring and continuity of care with periodic physician evaluations as defined by agreement with supervising physicians.
• Prepares prescription orders, administers medications/injections, sutures wounds, and provides wound care.
• Educates patients and families on health, medications, treatment instructions, and community resources.

The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on‑call coverage on a rotational basis, as required.

• Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation. 25%
• Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. 15%
• Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10%
• Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10%
• Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10%
• Triages patient telephone calls, provides monitoring and continuity of care10%
• Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.10%
