Job summary

Allegheny Health Network seeks a Nurse Practitioner to deliver primary care in a physician office setting, with weekend and on-call coverage as needed. You will assess patients, develop care plans, order tests, and document medical records within the CRNP scope.

You will collaborate with physicians, educate patients and families, triage calls, and participate in quality improvement initiatives while maintaining regulatory compliance including HIPAA.

Qualifications

• Master's degree in nursing (MSN) from an approved program.
• National Board Certification for Nurse Practitioners.
• Practicing-state licensure as a Nurse Practitioner.

Responsibilities

• Provide patient care including recording history and physical, developing therapeutic plans, ordering tests, interpreting results, and documenting medical records.
• Coordinate patient care with patients, families, community services, and interdisciplinary team; involve supervising physician as needed.
• Monitor patient care and arrange periodic physician evaluations as defined in the supervising agreement.
• Prepare prescriptions for medications and controlled substances; administer medications and injections; perform wound care.
• Educate patients and families on health, medications, and treatment instructions; refer to community resources as appropriate.
• Triage telephone calls and provide ongoing care and monitoring.
• Participate in quality improvement initiatives and ensure regulatory compliance.
• May perform other duties related to outpatient care in the specialty.
• Other duties as assigned.

GENERAL OVERVIEW:

This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required.

ESSENTIAL RESPONSIBILITIES

• Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.25%

• Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.15%

• Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).10%

• Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care10%

• Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.10%

• Triages patient telephone calls, provides monitoring and continuity of care10%

• Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.10%

• May perform other duties such as manage outpatient concerns and any other duty specific to specialty.10%

• Other duties as assigned or requested.

QUALIFICATIONS:

• Master's degree MSN from the practicing State Board of Nursing Approved Master's Program

• National Board Certification for Nurse Practitioners through an accredited association

• Practicing-state licensure as a Nurse Practitioner
