Ennoble Care is seeking an experienced Advanced Provider (NP/PA) to deliver primary and palliative care to homebound patients in Montgomery County, MD, on a full-time basis. You will apply medical principles under a physician’s supervision and care for a diverse patient population with chronic conditions.

Ideal candidates are board-certified in Maryland with strong clinical skills, a patient-centered approach, and the ability to work independently in a home-based setting.

Qualifications

• Board certification as NP or PA in Maryland.
• Strong clinical skills with accurate assessment and diagnosis.
• Positive demeanor and strong people skills.
• 1–2 years in primary care with geriatric patients; hospice experience is a plus.

Responsibilities

• Deliver high-quality primary and palliative care to homebound patients in Montgomery County.
• Conduct physical exams, review histories, and craft care plans.
• Communicate effectively with office staff and clinical operations platforms.
• Provide patient-centered care to a panel of patients.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostics as needed and prescribe medications and necessary interventions.

Purpose:

We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Montgomery County, MD region on a full-time basis. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

What You'll Do:

• Provide high quality care to primary and palliative patients in and around Montgomery County.
• Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
• Effectively communicate with office staff and clinical operations across various internal platforms
• Provide high quality, patient centered care to a panel of patients
• Develop impressions, diagnosis, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:

• Board Certified, with a valid NP or PA certification in the state of Maryland
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem solve and operate independently1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Compensation:

• $120,000- $140,000+ (depending on experience and other incentives) (full Time)
