As a Nurse Practitioner or Physician Assistant, you will be a vital member of our interdisciplinary Field Team, delivering in-home and community-based primary care to enhance the quality of life for our patients.

Job Details

• Employment Type: Local Contract
• Duration: 13 weeks
• Hours Per Week: 40
• Shift: 8 Hour Days, 5 shifts per week; No Weekends
• Start Date: ASAP
  
  

Responsibilities

• Deliver flexible clinical care, both in-person at patients' homes and virtually
• Provide compassionate, individualized, and holistic patient care, including comprehensive primary care, urgent care, and chronic condition management
• Collaborate with a multidisciplinary care team to deliver transitions of care support and complex care management
• Participate in professional development opportunities through clinical education programming
• Participate in after-hours on-call coverage as needed
  
  

Job Requirements

• Graduation from an accredited health professions school as a Nurse Practitioner or Physician Assistant
• Active national board certification
• Active licensure in the state of California
• Active DEA licensure and state-specific controlled substance registration, as applicable
• Certification in Basic Life Support (BLS)
• Ability to travel as needed with proof of liability and property damage insurance on vehicle used
• Minimum of 2 years of experience in primary care or a related field
