Job summary

Ennoble Care is seeking an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Hudson County, NJ. You will perform medical assessments, develop treatment plans, and collaborate with a physician supervisor to deliver comprehensive care in the patient’s home.

The role emphasizes patient-centered care, strong clinical skills, and the ability to work independently in a home-care setting, with a flexible schedule and supportive in-house clinical team.

Qualifications

• Board Certified with an NP or PA license in New Jersey.
• Strong clinical skills with the ability to assess and diagnose.
• Positive demeanor and strong people skills.
• Ability to problem-solve and work independently.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality primary and palliative care in Hudson County, NJ.
• Perform physical examinations and develop care plans.
• Communicate effectively with office staff and clinical operations.
• Deliver patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.

Purpose:

We are looking for an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Hudson County, NJ region. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

What You'll Do:

• Provide high quality care to primary and palliative patients in and around (Hudson County, NJ)
• Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
• Effectively communicate with office staff and clinical operations across various internal platforms
• Provide high quality, patient centered care to a panel of patients
• Attend quarterly medical staff meetings
• Develop impressions, diagnosis, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:

• Board Certified, with a valid NP or PA certification in the state of New Jersey
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Salary Range:
