Primary Responsibilities:
  1. Conduct in-home assessments, including evaluating medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform evidence-based screenings and point-of-care testing as appropriate.
  3. Identify diagnoses for care management and treatment planning.
  4. Document current and past medical conditions based on assessments.
  5. Communicate findings to inform the member’s PCP about potential care gaps.
  6. Identify urgent/emergent issues and intervene accordingly.
  7. Educate members on disease processes, medications, and adherence.

Required Qualifications:
  1. Active NP or PA license or ability to obtain it by start date, free of disciplinary actions.
  2. Active ANCC or AANP certification in Family, Geriatrics, Adult, or Adult-Gerontology primary or acute care, or ability to obtain certification/license in the state of assignment.
  3. Physical ability to perform assessments, including lifting 30 pounds, navigating stairs, and sitting/standing/kneeling.
  4. Reliable transportation for home visits.
  5. Fluency in Spanish.
