Job description

Job Opportunity: Nurse Practitioner for HouseCalls Program

Compensation: $40,000 Student Loan Repayment OR $40,000 Sign-on Bonus for eligible individuals who have not previously participated in this program.

Schedule: Flexible provider schedules between 20-40 hours per week, with no on-call, weekends, or holidays required.

Optum Home & Community Care, part of UnitedHealth Group, is innovating in healthcare by creating an integrated care model that addresses patients' physical, mental, and social needs, enabling access and navigation of care anytime, anywhere.

As part of our HouseCalls team, you will conduct annual clinical assessments in patients' homes, helping to identify health risks and coordinate follow-up care to enhance well-being. Our model is nurse practitioner-led, focusing on preventive care to keep patients healthy and satisfied, with a 99% member satisfaction rate in 2021.

This role offers work-life balance through flexible scheduling over a 7-day week, with no requirement for on-call, weekends, or holidays.

Primary Responsibilities:
  1. Conduct comprehensive in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform screenings and point-of-care testing as appropriate.
  3. Identify diagnoses for care management and treatment planning.
  4. Document findings to inform the patient's primary care provider about care gaps.
  5. Recognize urgent/emergent situations and respond accordingly.
  6. Educate patients on disease management, medications, and compliance.
Minimum Qualifications:
  • Active Nurse Practitioner (NP) or Physician Assistant (PA) license or ability to obtain by start date, with no disciplinary restrictions.
  • Active national certification (ANCC or AANP) in Family, Geriatrics, Adult, or Adult-Gerontology primary or acute care, or ability to obtain it.
  • Physical ability to perform assessments, including lifting up to 30 pounds, navigating stairs, and sitting/standing/kneeling.
  • Reliable transportation for home visits.
  • Fluency in Spanish.
Preferred Qualifications:
  • At least 1 year of clinical experience in your highest level of education.
  • Potential to obtain additional licensure in other regions.
  • Experience in geriatrics, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel up to 5% of the time, with additional compensation incentives.

Additional Notes: Employees must remain in the role for at least 12 months; leaving earlier will cease student loan benefits. Full benefits are available, including salary range for New York residents ($88,000 - $173,200), and comprehensive health and retirement plans.
