Job description

Job Opportunity: Nurse Practitioner - Home & Community Care

Compensation and Schedule: $40,000 student loan repayment or $30,000 sign-on bonus for eligible individuals. Flexible provider schedules available between 20-40 hours per week.

Role Overview: As a team member of our HouseCalls program, you'll conduct annual clinical assessments at patients' homes, helping reduce health risks and coordinate follow-up care, with a high patient satisfaction rate.

Work Environment: Enjoy work-life balance with flexible scheduling, no on-call, weekends, or holidays required.

Responsibilities:

  1. Conduct in-home assessments including medical history, physical exams, and vital signs.
  2. Perform screenings and point of care testing as appropriate.
  3. Identify diagnoses and manage care plans.
  4. Communicate assessment findings to primary care providers.
  5. Identify urgent situations and intervene.
  6. Educate members on health topics.

Qualifications:

Required:
  • Active NP license or ability to obtain one by start date.
  • National certification in Family, Geriatrics, or Adult Primary Care.
  • Physical ability to perform assessments and reliable transportation.
Preferred:
  • 1+ year clinical experience.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care experience.
  • Ability to travel up to 5% of the time.
  • Fluency in additional languages such as Spanish, Cantonese, Korean, Vietnamese, or Polish.
