The GoFollowUp NP or PA answers incoming calls from patients/customers and conducts outreach to patients to relay details on patient visit. Responsible for answering inquiries and questions, handles complaints, troubleshoots problems and provides information for patients/customers. Job Requirements include graduation from an accredited Nurse Practitioner or Physician Assistant program, 3+ years of clinical practice experience (Emergency Medicine/Urgent Care preferred), and active Nurse Practitioner or Physician Assistant State License. Additional skills required include proficiency in relevant computer applications/electronic medical records, Microsoft Word and Excel, exceptional communication skills, and knowledge of call center telephony and technology. Core competencies include collaboration, innovation, diversity and inclusion, integrity, and accountability. Essential functions include reviewing and managing patient results for labs and imaging, relaying results and care plans to patients, responding to escalated questions and patient inquiries, identifying and prioritizing patient needs, and conducting patient follow-up outreach. The job description is subject to revision by management and may include other duties as assigned.
