Job summary

Live Well Homecare is seeking a clinician to conduct comprehensive in-home health assessments for adults and seniors. Assessments include medical history review, medication adherence, social factors, and tests, with documentation of findings at the patient's residence or in Skilled Nursing Facilities.

The role requires NP/PA licensure, prescriptive authority, CPR/BLS/ACLS, and at least 1 year of NP/PA experience. Travel is extensive, and a valid driver's license is required.

Qualifications

• Master’s degree and NP/PA licensure required.
• Current prescriptive authority with relevant certifications.
• CPR/BLS/ACLS certifications required.
• Minimum 1 year NP/PA experience.
• Willingness to travel extensively, including overnight stays.

Services are provided at members' residences, including homes and Skilled Nursing Facilities.
