What to Expect (Job Responsibilities):
Responsibilities
- Establish and maintain effective relationships with patients, caregivers, and interdisciplinary team members
- Perform comprehensive assessments on newly enrolled patients and provide ongoing care, including condition changes
- Develop and update patient management care plans as needed
- Conduct post-discharge visits to ensure patient stability aligns with their care goals
- Participate in quality initiatives and campaigns to enhance care management and delivery
What is Required (Qualifications):
Qualifications
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current, unrestricted NP or PA licensure in Washington state
- Active, unrestricted RN licensure in Washington state for Nurse Practitioners
- Reliable transportation and compliance with Optum Motor Vehicle Safety policy
Preferred Qualifications
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, or similar settings
- Knowledge of long-term care, community resources, and patient care delivery systems
- Familiarity with CPT, ICD-10, and HCPC codes
- Awareness of NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts
