What to Expect (Job Responsibilities):

Responsibilities
  • Establish and maintain effective relationships with patients, caregivers, and interdisciplinary team members
  • Perform comprehensive assessments on newly enrolled patients and provide ongoing care, including condition changes
  • Develop and update patient management care plans as needed
  • Conduct post-discharge visits to ensure patient stability aligns with their care goals
  • Participate in quality initiatives and campaigns to enhance care management and delivery

What is Required (Qualifications):

Qualifications
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • Current, unrestricted NP or PA licensure in Washington state
  • Active, unrestricted RN licensure in Washington state for Nurse Practitioners
  • Reliable transportation and compliance with Optum Motor Vehicle Safety policy
Preferred Qualifications
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, or similar settings
  • Knowledge of long-term care, community resources, and patient care delivery systems
  • Familiarity with CPT, ICD-10, and HCPC codes
  • Awareness of NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts
