Job description
Overview

What to Expect
  • Establish and maintain effective relationships with patients, caregivers, and interdisciplinary team members
  • Perform comprehensive assessments on newly enrolled patients and provide ongoing care, including follow-up visits
  • Develop and update patient management care plans based on changes in condition or hospitalization
  • Conduct post-discharge visits to ensure patient stability aligns with their care goals
  • Participate in quality initiatives and HEDIS measure campaigns to enhance care management and delivery
Required Qualifications
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • Current, unrestricted NP or PA licensure in Washington State
  • Active, unrestricted RN licensure in Washington State for Nurse Practitioners
  • Access to reliable transportation and compliance with the Optum Motor Vehicle Safety policy
  • Ability to perform basic procedures in episodic nursing care
Preferred Qualifications
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar settings
  • Knowledge of long-term care, community resources, and cost-effective alternatives for patient care delivery
  • Familiarity with CPT, ICD-10, and HCPC codes
  • Understanding of clinical standards of care and NCQA requirements
  • Awareness of CMS guidelines, Medicaid/Medicare contracts, and benefit systems
