What to Expect (Job Responsibilities)
  • Establish and maintain effective relationships with patients, caregivers, and healthcare providers
  • Conduct comprehensive assessments for newly enrolled patients and provide ongoing care
  • Develop and update patient management care plans based on changes in condition
  • Perform basic procedures such as IV placement, blood draws, and wound care
  • Participate in quality initiatives to promote high standards of care
What is Required (Qualifications)
  • Certification through the American Academy of Nurse Practitioners (AANP), ANCC, or NCCPA
  • Current, unrestricted NP or PA licensure in Washington state
  • Active, unrestricted RN licensure in Washington state for Nurse Practitioners
  • Access to reliable transportation and compliance with Optum’s Motor Vehicle Safety policy
How to Stand Out (Preferred Qualifications)
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or similar settings
  • Familiarity with long-term care and community resources
  • Knowledge of CPT, ICD-10, and HCPC codes
  • Understanding of clinical standards of care and NCQA requirements
