A healthcare recruitment agency is seeking a dedicated Nurse Practitioner or Physician Assistant to join their Family Medicine team in South Carolina. The role involves providing comprehensive primary care across several outpatient sites, emphasizing patient-centered care and chronic disease management. The ideal candidate will have at least 2 years of experience in a primary care setting and must be licensed to practice in South Carolina. Qualificações

• Graduate of an accredited Advanced Practice Registered Nurse/Nurse Practitioner program or Physician Assistant program.
• Minimum of 2 years of experience in a primary care or family medicine setting required.
• Active and unrestricted South Carolina medical license.
• Certified or eligible for certification in area of specialty (FNP, PA-C).

Responsabilidades

• Provide comprehensive primary care services to patients across the lifespan.
• Travel regularly to assigned family medicine sites within the designated counties.
• Deliver high-quality, evidence-based care with an emphasis on wellness, prevention, and chronic condition management.
• Collaborate effectively with physicians, nurses, and other team members to coordinate patient care.
• Educate patients and families about treatment plans, medications, and health maintenance.
• Document patient encounters accurately and timely in the electronic health record system.

We are seeking a dedicatedand experienced Nurse Practitioner or Physician Assistant to join our FamilyMedicine team as a floating provider. This role will support patient careacross multiple outpatient family medicine sites in Abbeville, Greenwood,Laurens, McCormick, and Saluda counties in South Carolina. The ideal candidatewill be passionate about patient-centered care, preventive health, and chronicdisease management, while building strong, trusting relationships with patientsand their families.

Key Responsibilities:

• Provide comprehensiveprimary care services to patients across the lifespan.
