Minimum Requirements

• Graduate from an accredited program with an advanced degree.
• Unrestricted license to practice in Indiana.
• Minimum of two (2) years of experience; neurology experience preferred.
• Ability to obtain prescriptive privileges and maintain current privileges, with a collaborative practice agreement with a designated physician.
