Role Summary

The primary purpose of the APP is to deliver provider-level direct patient care within the Post-Acute Care setting.

Responsibilities

1. Conduct clinical visits, including episodic and re-evaluation visits.
2. Evaluate and treat chronic and co-morbid illnesses.
3. Assess and respond to changes in patient conditions.
4. Review medications, orders, and diagnostic tests, collaborating with physicians as needed.
5. Coordinate care with physicians and other healthcare disciplines.
6. Perform Annual Wellness Visits and other assessments as requested.
7. Assist with facility-wide health issues and collaborate with interdisciplinary teams.
8. Communicate with families and attend conferences when possible.
9. Perform other duties as assigned.

Qualifications

1. Valid nurse practitioner or physician assistant license in North Carolina and current board certification.
2. Master’s Degree in Nursing or Physician Assistant Studies.
3. Strong leadership, interpersonal, and problem-solving skills.
4. Excellent communication skills.
5. Traits of punctuality, organization, and teamwork.
