Job summary

IconicCare is seeking a motivated physiatrists to provide independent care in contracted skilled nursing facilities. You will round 1–3 facilities 1–3 days per week delivering specialized physiatry services to 25+ patients daily who struggle to participate in therapy due to pain or functional limitations.

You will lead assessments, create treatment plans, and work with facility teams to optimize rehab outcomes.

Qualifications

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
• A genuine passion for providing professional, personalized care in an autonomous environment.
• Strong communication skills, with the ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting

Responsibilities

• Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
• Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
• Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

We’re Hiring:

We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.

Day-to-Day Responsibilities

As a physiatrist, you will round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
