Job summary

Cognizant Behavioral Health Services in Chadds Ford, PA, seeks an Advanced Practice Provider (PMHNP or PA-C) to join our outpatient behavioral health team. You will conduct psychiatric evaluations, manage medications, and provide ongoing treatment in a collaborative care setting.

The ideal candidate has NP/PA training, PMHNP or PA-C certification, PA license, and DEA registration, with 3 years of experience in behavioral health preferred.

Qualifications

Responsibilities

We are seeking an Advanced Practice Provider—Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant-Certified (PA-C)—to join our growing practice in Chadds Ford, Pennsylvania.

Position Overview

The APP will provide psychiatric evaluations, medication management, and ongoing behavioral health treatment in a supportive outpatient environment. The ideal candidate will demonstrate sound clinical judgment, strong communication skills, and a commitment to delivering compassionate, individualized care.

Key Responsibilities

• Conduct comprehensive psychiatric evaluations and assessments.
• Diagnose and treat a variety of behavioral health conditions.
• Develop individualized treatment plans based on each patient’s clinical needs, history, and goals.
• Prescribe and manage psychiatric medications within the provider’s scope of practice.
• Monitor patient progress, medication effectiveness, and potential side effects.
• Provide patient and family education regarding diagnoses, medications, and treatment recommendations.
• Collaborate with psychiatrists, therapists, and other members of the treatment team.
• Maintain accurate and timely documentation in the electronic health record.
• Coordinate care with primary care providers, specialists, and community resources when appropriate.
• Follow all clinical, ethical, regulatory, and organizational standards.

Qualifications

• Graduate of an accredited nurse practitioner or physician assistant program.
• Current certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant-Certified (PA-C).
• Current or eligible Pennsylvania professional license.
• Current or eligible DEA registration, as required for the position.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while contributing to a collaborative clinical team.
• 3 years of experience.
