Job summary

Cognizant Behavioral Health Services in Chadds Ford, PA is seeking an Advanced Practice Provider—PMHNP or PA-C—to join our outpatient behavioral health team. The APP will perform psychiatric evaluations, manage medications, and provide ongoing behavioral health care in a collaborative setting.

Ideal candidates will have current PMHNP or PA-C certification, PA license in PA, and DEA registration, with at least 3 years of experience and strong communication skills.

Qualifications

• Graduate of an accredited nurse practitioner or physician assistant program.
• Current certification as PMHNP or PA-C.
• Current or eligible Pennsylvania professional license.
• Current or eligible DEA registration, as required for the position.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while contributing to a collaborative clinical team.
• 3 years of experience.

Responsibilities

• Conduct comprehensive psychiatric evaluations and assessments.
• Diagnose and treat a variety of behavioral health conditions.
• Develop individualized treatment plans based on each patient’s clinical needs, history, and goals.
• Prescribe and manage psychiatric medications within the provider’s scope of practice.
• Monitor patient progress, medication effectiveness, and potential side effects.
• Provide patient and family education regarding diagnoses, medications, and treatment recommendations.
• Collaborate with psychiatrists, therapists, and other members of the treatment team.
• Maintain accurate and timely documentation in the electronic health record.
• Coordinate care with primary care providers, specialists, and community resources when appropriate.
• Follow all clinical, ethical, regulatory, and organizational standards.
