Job summary

Twin Cities Physicians in Minnesota is seeking an Advanced Practice Wound Care NP (AGNP/CNP) to provide geriatric wound care across assisted living, memory care, and long-term care communities.

In this full-time role, you will conduct advanced wound assessments, develop treatment plans, manage medications, and collaborate with physicians and care teams to improve healing outcomes. Travel within the Twin Cities Metro is required.

Qualifications

• Active Minnesota NP license; AGNP preferred.
• Wound care and geriatrics experience preferred.
• Strong clinical judgment and ability to manage medically complex patients.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing wound management.
• Conduct routine rounding visits within assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Schedule: Full-Time | Monday through Friday

• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
