Job summary

Twin Cities Physicians is seeking a Wound Care Nurse Practitioner to lead advanced wound care for post-acute and senior living populations. You’ll assess, plan, and manage complex wounds across multiple facilities, coordinating with doctors, nurses, and therapists.

Ideal candidates hold an Minnesota NP license (AGNP preferred) with wound care and geriatrics experience, and must travel across the Twin Cities Metro Area. Full benefits and competitive compensation offered.

Responsibilities

• Provide advanced wound care assessments and treatment planning for complex wounds.
• Conduct routine rounding visits across post-acute, memory care, and long-term care communities.
• Manage pressure injuries, ulcers, and skin integrity issues.
• Collaborate with physicians, nurses, therapists, and facility teams.
• Support chronic disease management and preventive care initiatives.
• Help reduce avoidable hospital readmissions through proactive follow-up.

Job Title: Wound Care Nurse Practitioner (AGNP/CNP) | Post-Acute & Senior Living Rounding

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Schedule: Full-Time | Monday through Friday

What You’ll Be Doing

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout post-acute, assisted living, memory care, and long-term care communities.
• Manage pressure injuries, surgical wounds, diabetic ulcers, skin integrity concerns, and related conditions.
• Collaborate closely with physicians, nurses, therapists, and facility teams.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help reduce avoidable hospital readmissions through proactive intervention and follow-up.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, post-acute care, geriatrics, primary care, skilled nursing, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
