Job description

We are looking for NP and PA to join our team in conducting patient centered care in Medicare patients' homes.

The Nurse Practitioner or Physician Assistant will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments as required.

Competencies:
  • Performs initial patient screenings, physical examinations, and creates assessment plans.
  • Addresses urgent care needs of the patients as needed.
  • Orders, interprets, and analyzes diagnostic tests and determines the need for follow-up appointments and further treatment options.
  • Prescribes and manages medications and treatments as appropriate, including but not limited to psychopharmacological agents.
  • Delivers palliative care, and when needed, end-of-life care to patients and families.
  • Delivers quality care while maintaining all company metrics pertaining to appropriate documentation, coding, HEDIS measures, engagement of patients, and managing medical expenses.
Qualifications:
  • Experience in family practice, geriatrics, hospice care, palliative care, hospitalist, ICU, SICU, ER.
  • Master's Degree.
  • Valid physician assistant or NP license and Board Certification.
  • NPI number.
  • BLS/ACLS certification.
  • Valid driver's license, auto insurance, and willingness to drive to patients' homes/patient locations.
  • Salary: $120,000 - $140,000 plus DOE; excellent benefit package including 4 weeks PTO and more.
  • No COVID vaccinations required.
