The APPs work with the staff on the Transplant unit at MGH, responding to urgent issues and acting as a resource for staff. They are the Responding Clinician for patients admitted to the Transplant Surgical Service throughout the hospital, including patients that have undergone liver and/or kidney transplants, as well as those patients who are admitted for a wide variety of other surgical or medical concerns (i.e., infection, rejection, UTI, and/or heart failure). They collaborate with physicians and other healthcare professionals to provide comprehensive care to patients including contributing to daily multidisciplinary Transplant Rounds. The APPs order, interpret, and evaluate appropriate labs and diagnostic tests, maintain accurate and complete medical records in EPIC. They also coordinate discharge planning with collaboration of services, prescribing medications, arranging post-hospitalization follow-up appointments, and handing off to the outpatient Transplant Clinic. EXPERIENCE: • 3-5 years of in-patient Med/Surg or Acute Care experience, preferred. • Recent experience as an APP with demonstrated clinical competence and professional accountability preferred. • Candidates with related experience (Transplant, ICU or Inpatient experience) will be considered EDUCATION: • Master’s degree is required. LICENSES, CERTIFICATIONS, and/or REGISTRATIONS: • Current licensure as a Nurse Practitioner or Physician Assistant in the Commonwealth of Massachusetts. Essential Functions
-Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting
-Performs complete histories and physical examinations
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings
-Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries
-Performs bedside procedures as are appropriate to the patient population
-Orders medications and writes prescriptions according to organizational and regulatory policies and procedures
