Job Overview

The Nurse Practitioner or Physician Assistant will oversee the Community Health Worker (CHW) team, evaluate assessments, and provide holistic, quality care to members enrolled in managed care plans. Responsibilities include closing care gaps, addressing social needs, re-engaging members with primary care, and supporting improved quality of life and reduced healthcare costs. The role is primarily home-based with supplemented telehealth visits for complex medical populations.

Key Responsibilities
  • Oversee and evaluate assessments by the CHW team.
  • Provide comprehensive care, addressing medical, mental, and social health issues.
  • Coordinate with healthcare providers and community resources.
  • Manage and lead a team including Community Health Workers, Patient Care Navigators, and other staff.
  • Document patient interactions accurately and timely.
Qualifications
  • California licensure as a Nurse Practitioner or Physician Assistant.
  • National Board Certification.
  • Graduation from an accredited Master’s or Doctoral program.
  • 1-3 years healthcare management and patient care experience preferred.
  • Experience in home health care for complex, homebound patients preferred.
  • Bilingual abilities (e.g., Spanish, Chinese, Creole) preferred.
Working Knowledge
  • Proficiency in EMR systems, Outlook, and MS Office.
  • Understanding of Medicare, Medicaid, and community resources.

Work Environment & Physical Demands
