Job summary

SSM Health is seeking a clinician in our Waupun occupational health clinic to provide patient care in a 100% outpatient setting. The role focuses on pre-employment and work-related physicals and will partner with employer contracts to develop population health programs.

This full-time opportunity (32 hours/week) includes a full benefits package, no nights or weekends, access to EPIC EMR, and on-site labs/imaging at SSM Health Waupun Memorial Hospital.

Qualifications

• Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) program.
• Ability to obtain PA/NP licensure and DEA license in Wisconsin.
• Preferred certifications - DOT medical examiner, NIOSH, CAOHC.

Responsibilities

• Provide pre-employment and work-related physicals.
• Develop population health programs in partnership with employers.
• Deliver patient-centered care in an outpatient clinic setting with access to on-site labs/imaging.
• Utilize EPIC EMR to document patient encounters.

Your Qualifications:

• Experience strongly preferred
• Detail-oriented

• Ability to obtain PA/NP licensure and DEA License in Wisconsin
  • NPs require APRN
• Preferred certifications - will need to be obtained
  • Medical Examiner certification for DOT physicals
