Job summary

Rochester Regional Health is seeking a Nurse Practitioner in Geriatrics to deliver comprehensive care to older adults across nursing homes, assisted living, and independent living settings in the Rochester area. Qualifications

• RN license in NYS (required)
• NP certification in NYS (required)
• MSN degree or equivalent

Responsibilities

• Provide primary and acute care to older adults, including assessment, diagnosis, treatment, medication management, and follow-up
• Manage acute and chronic conditions across nursing home, assisted living, and independent living settings
• Collaborate with physicians, nursing, facility staff, and other care team members to coordinate comprehensive patient care
• Monitor patient progress, adjust treatment plans as appropriate, and maintain timely and accurate documentation

As a Nurse Practitioner in Geriatrics, you’ll provide comprehensive care to older adults across nursing home, assisted living, and independent living settings. You’ll manage a variety of acute and chronic conditions while developing meaningful, long-term relationships with patients.

LOCATION: Nursing Homes and Assisted Living Facilities throughout the Rochester Area
DEPARTMENT: Geriatrics
SCHEDULE: Days, Monday-Friday, No Weekends or Call

PHYSICAL REQUIREMENTS

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision‑making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.
