Job summary

Enhabit Home Health & Hospice is seeking a Nurse Practitioner to provide medical care to hospice patients and determine eligibility, pain and symptom management. You will be part of the interdisciplinary team delivering compassionate hospice care across the organization.

Applicants must be licensed as an NP in the practicing state, with knowledge of Medicare COPs and robust technology skills including mobile devices. CPR certification is required; driving and transportation are part of the role.

Qualifications

• Must be NP licensed in the state of practice and graduate of an approved school.
• Knowledge of Medicare conditions of participation and regulatory requirements.
• Basic technology skills including mobile device operation.

Responsibilities

• Provide medical care to hospice patients.

Responsibilities

Provide medical care to hospice patients. Determine hospice eligibility and pain and symptom management. Participate in the care of the hospice patient as a member of the interdisciplinary team

Qualifications

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
