Qualifications

• Mindestens 2 Jahre klinische Erfahrung in Geriatrie, Erwachsenen- oder Familienmedizin.
• Erfahrung mit ICD-10, HCC-Codierung und wertbasierten Pflege-Modellen ist bevorzugt.

Join to apply for the Nurse Practitioner (NP), Adult and/or Gerontology role at Personalized Health Partners.

PHP Nurse Practitioners are passionate about delivering quality care to seniors and residents in healthcare settings such as skilled nursing facilities, assisted living, independent living communities, and via telehealth.

We are currently recruiting Nurse Practitioners for Addison Healthcare Center in Masury, OH. Work hours are Monday - Friday, 8 am - 5 pm, salaried, with some weeks exceeding 40 hours. No call, weekends, or holiday coverage required. Must be available to work five days a week.

The role involves providing high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses, and collaborating with the healthcare team and patients’ families to develop care plans.

Responsibilities include:

• Assessing, diagnosing, and managing conditions.
• Providing health education.
• Developing and communicating treatment plans.
• Ordering and interpreting tests; adjusting treatments.
• Documenting in EMR software.
• Collaborating with healthcare teams and supporting strategic initiatives.
• Ensuring excellent patient experience and compliance with regulations.

Qualifications:

• MSN, DNP, or NP certificate from an accredited program.
• Active certification (AANP or ANCC) and unrestricted state license.
• DEA license and US work authorization.
• At least 2 years of clinical experience in geriatrics, adult, or family practice.

Preferred:

• Geriatrics training or experience.
• Experience with ICD-10, HCC coding, and value-based care models.

Skills required:

• Strong communication, detail orientation, and time management skills.
• Ability to work independently and in teams, with a customer service focus.
