Job summary

Legacy Medical is seeking a Nurse Practitioner to join our on-site primary care team in Cheyenne & Laramie, Wyoming. This role includes developing diagnoses, treatment plans, and preventive care for residents in senior living communities.

A sign-on bonus is offered, and employment is contingent on background check and drug screen. You will collaborate with a multidisciplinary team to deliver high-quality patient outcomes, maintain thorough health records, and promote best practices in infection

Responsibilities

• Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses
• Build a rapport with the patient and explain condition and course of action effectively through written and oral communication
• Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines
• Promote preventive care and health maintenance including annual physicals, positive health behaviors and self-care through both formal and individual education and counseling
• Maintain complete and detailed records of patients' health care plans and prognoses
• Perform quality control and maintain a high standard in relation to infection control mechanisms for patient safety
• Work collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients
