Job summary

Crossroads Hospice & Palliative Care in Canton, Ohio seeks a Nurse Practitioner to provide compassionate, patient- and family-centered care across the end-of-life spectrum. You will assess, plan, and coordinate care with a focus on comfort, dignity, and informed decision-making.

This community-based role requires travel between visits, collaboration with a multidisciplinary team, and adherence to licensing and regulatory requirements.

Qualifications

• 2 years of Nurse Practitioner experience in an acute care environment.
• Home Health, Palliative Care, or Hospice Experience Required.
• Masters of Science in Nursing.
• Active unencumbered OHNP license.
• Current driver's license and auto liability coverage.
• Detailed assessment skills as part of this role require recertification for Hospice patients.
• Interest in a community-based role that requires travel between patient visits.
• DEA number Required.
• National Provider Identifier (NPI) Required.

Responsibilities

• Provide comprehensive palliative and hospice NP care to patients in their homes and community settings.
• Partner with families to support goal-concordant care, symptom management, and advanced care planning.
• Collaborate with a multidisciplinary team to ensure seamless, high-quality care across visits.
