Job summary

Crossroads Hospice & Palliative Care in Akron, OH seeks a Nurse Practitioner to provide compassionate, high-quality hospice and palliative care. You will perform patient assessments, develop and adjust plans of care, and coordinate with an interdisciplinary team.

Travel between patient visits is required to support families in our community. A MSN, OH NP license, DEA/NPI numbers, and home health/palliative care experience are essential. This role offers competitive pay and benefit options.

Qualifications

• 2 years of Nurse Practitioner experience in an acute care environment.
• Home Health, Palliative Care, or Hospice Experience Required.
• Masters of Science in Nursing.
• Active unencumbered OHNP license.
• Current driver's license and auto liability coverage.
• Detailed assessment skills as part of this role require recertification for Hospice patients.
• Interest in a community-based role that requires travel between patient visits.
• DEA number Required.
• NPI Required.

Responsibilities

• Provide comprehensive NP care to hospice/palliative patients.
• Collaborate with physicians, nurses, and care coordinators to develop and adjust care plans.
• Educate patients and families about disease progression, treatment options, and comfort measures.
• Document assessments, orders, and care plans accurately and in a timely manner.
