Job summary

Crossroads Hospice is seeking an experienced Nurse Practitioner to join our Ohio-based palliative care team. You will provide compassionate, patient-centered care, travel between visits, and collaborate with physicians and care teams.

Requirements include 2+ years NP experience in acute care, MS in Nursing, OH NP license, DEA/NPI, and a valid driver’s license; travel is required. Benefits include health, dental, vision, 401k, PTO, CE reimbursement, and opportunities for growth.

Position Qualifications

• 2 years of Nurse Practitioner experience in an acute care environment
• Home Health, Palliative Care, or Hospice Experience Required
• Masters of Science in Nursing
• Active unencumbered OH NP license
• Current driver's license and auto liability coverage
• Detailed assessment skills as part of this role require recertification for Hospice patients
• Interest in a community-based role that requires travel between patient visits
• Drug Enforcement Agency (DEA) number Required
• National Provider Identifier (NPI) Required
