The Role
We are seeking a dedicated and experienced Nurse Practitioner in Neuromuscular Neurology to join our innovative virtual care team. This full-time, remote position offers the opportunity to deliver high-quality, patient-centered care to individuals living with complex neuromuscular diseases such as ALS, SMA, and other motor neuron disorders. You’ll work collaboratively with Synapticure neurologists and an integrated care team to ensure each patient receives proactive, personalized, and compassionate care, regardless of their geographic location.
As a core member of our clinical team, you’ll manage virtual patient visits, develop evidence-based care plans, and support patients across all stages of their disease journey—including symptom management, palliative support, and clinical trial education. You’ll also contribute to evolving our care model and participate in interdisciplinary case discussions that drive high standards of quality and consistency across the care continuum.

Job Duties – What you’ll be doing
    • Provide direct virtual care for patients with neuromuscular diseases in partnership with Synapticure neurologists.
    • Conduct detailed medical histories, perform telehealth assessments, and manage medications and treatment plans.
    • Develop individualized, evidence-based care plans tailored to disease progression, comorbidities, and patient goals.
    • Collaborate closely with care coordinators, genetic counselors, therapists, and other specialists to deliver integrated care.
    • Support end-of-life planning, palliative care, and symptom management as needed.
    • Document visits, coordinate care, and track clinical outcomes using Synapticure’s digital health platform and EMR.
    • Participate in regular multidisciplinary team meetings and help refine care pathways and clinical protocols.
    • Educate patients and caregivers about disease progression, treatments, clinical trials, and available resources.
Requirements – What we look for in you
    • Active Nurse Practitioner license and national certification (AGNP, FNP, or ACNP).
    • Eligible to practice in multiple states; IMLC licensure a plus.
    • Minimum of 5 years of clinical experience with neuromuscular disorders such as ALS or related conditions.
    • Proficiency with telehealth and virtual care platforms.
    • Ability to deliver compassionate, patient-centered care to individuals with high-complexity needs.
    • Strong interdisciplinary collaboration and communication skills.
    • Comfortable working in a mission-driven, fast-paced startup environment.
Preferred Qualifications
    • Experience supporting clinical trials, genetic counseling, or palliative care.
    • Previous work within a multidisciplinary care model.
    • Prior experience in digital health or remote care delivery.
    • Licensure in multiple states or current IMLC status.
