Job Summary: The National After-Hours Call Team offers telephonic care during overnight, weekend, and holiday hours, ensuring high-quality, continuous care for residents across multiple states. This home-based role involves providing telephonic and electronic medical record-based care, managing acute and chronic conditions, and collaborating with physicians and specialists.

Essential Duties:

  1. Provide telephonic care and coordinate resident care between facilities, hospitals, and clinics.
  2. Utilize EMR to deliver acute care during after-hours shifts.
  3. Manage medical and behavioral health conditions, perform assessments, and document findings per CMS regulations.
  4. Attend mandatory training and adhere to practice guidelines.

Scheduling & Hours: Various shifts including weeknights, weekends, and holidays, with coverage from 5pm to 8am local time. The role involves working every other weekend and holiday shifts, with an expectation of 40 hours per week for full-time positions.

Qualifications:

  • Master's Degree in Nursing and current unrestricted NP license in your state.
  • National certification as ANP, FNP, or GNP.
  • Ability to obtain DEA licensure and prescriptive authority.
  • At least 3 years of NP experience, with a background in acute and chronic disease management, preferably in geriatrics or adult care.
  • Proficient in electronic medical records documentation.

Preferred Skills:

  • Experience in nursing homes or senior care settings.
  • Knowledge of geriatrics, chronic illnesses, and end-of-life discussions.
  • Strong communication and adaptability skills.
