Job Summary

The National After-Hours Call Team offers telephonic care to residents and facility staff during overnight, weekend, and holiday hours, ensuring high-quality, continuous care across multiple states. This home-based role involves providing virtual care to aging residents in various settings, with shifts covering weeknights, weekends, and holidays.

Essential Duties & Responsibilities
  • Handle calls on Curana’s telephonic platform to coordinate care between facilities, hospitals, and clinics.
  • Utilize EMR proficiently to provide acute care during overnight and weekend hours.
  • Deliver primary care, managing medical and behavioral conditions in collaboration with physicians or specialists.
  • Perform comprehensive assessments, document findings accurately, and ensure compliance with CMS regulations.
  • Attend all mandatory educational and training sessions.
Scheduling & Hours

Coverage includes weeknights (5pm-8am), weekends (every other weekend, 10-12 hour shifts), and holidays, with a full-time, salaried position requiring 40 hours weekly.

Qualifications
  • Master's Degree as a Nurse Practitioner
  • Current unrestricted NP license in your state and ability to obtain licenses in other required states within 60 days
  • National certification as ANP, FNP, or GNP
  • Ability to obtain DEA licensure and prescriptive authority
  • Experience in acute and chronic disease management, preferably with adults, families, or geriatrics
  • Minimum 3 years of NP experience
  • Proficiency in electronic medical records and documentation
Preferred Qualifications
  • Experience in nursing homes or senior care settings
  • Knowledge of geriatrics, chronic illness, and end-of-life discussions
  • Strong patient relationship skills and adaptability to change
