Adobe Population Health (APH) is seeking an In-Home Assessment Nurse Practitioner (IHA NP) to assess patient and family needs in their homes, plan and procure services, and coordinate care across the continuum. The role requires traveling within Las Vegas and Clark County, supporting quality of life and health literacy through in-home visits that can last up to an hour.

Both part-time and full-time opportunities are available.

POSITION PURPOSE

The In-Home Assessment Nurse Practitioner (IHA NP) is responsible for assessing patient/family psychosocial needs, planning, implementing, procuring services for, and evaluating care needs while meeting with members in their homes. As an IHA NP, your goal is to help improve the quality of life and health literacy by completing an in-home visit (that can last up to one hour) evaluating health status, medical history, current medications, social determinants, and other risks. The IHA NP helps to coordinate, monitor, and ensure information is shared in an appropriate and timely primary, across the continuum of care. The IHA NP will promote the effective utilization of tools and monitor healthcare resources. Both part-time and full-time opportunities are available. This position requires travel in Las Vegas, around Clark County, and neighboring areas. Mileage reimbursement is provided!

DUTIES & RESPONSIBILITIES
  • Travel up to 70% throughout designated regions, which may be updated based on business needs.
  • Conduct in-home assessments on identified patients, following established guidelines.
  • Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.
  • Identify urgent and emergent situations for proper intervention.
  • Educate members on topics related to health conditions and the continuation of care.
  • Identify diagnoses to be used in care management and active medical management.
  • Provide care coordination to alleviate identified needs related to behavioral,physical, and social issues.
  • Focus on addressing issues that will decrease medical costs and increasethe quality of life.
  • Develop positive relationships with clients, caregivers, providers, and other team members.
  • Managing physical health problems through the provision of education.
  • Communicate gaps in care to the member’s primary care physician if needed
  • Home safety evaluations.
  • Identify internal and external referral needs when available.
  • Compliance with all HIPPA regulations and maintain the security of protected health information (PHI).
SKILLS & QUALIFICATION
  • One (1) year of experience as an NP is preferred.
  • Experience in the care of the elderly in some geographic regions is preferred.
  • Experience in home health, physician’s office, or public health is preferred.
  • Adapts to changes in policies, procedures, new techniques, & additional responsibilities.
  • Good driving record .
  • Proof of insurance
  • Knowledge of HIPAA regulations.
  • Bilingual (English/Spanish) is preferred.

  • Master's Degree in Nursing is required
